Remarks: Total iron binding capacity (TIBC) is used for the differential diagnosis of anaemia, especially with hypochromia and low MCV. High TIBC with low saturation and low iron indicate uncomplicated iron deficiency, due to blood loss or inadequate iron intake Low Ferritin levels are the first indication of iron deficiency if inflammation is absent. In sideroblastic anemia TIBC may be normal to low, serum iron normal to high and saturation increased.
